  • Obverse: Blue design over green-yellow underprint. In the center, representation of the "Foundation of Caracas by Diego de Losada" surrounded by monks, indians and soldiers. On the right, inside an oval portrait of Simon Bolivar by unknown author. On the top, the issuer authority "BANCO CENTRAL DE VENEZUELA" «Central Bank of Venezuela» with two serial number in red below. In the upper left and bottom right, the value "CINCO" inside guilloche oval; In the upper right value "5" inside a circle guilloche; In the bottom left value "5" inside a guilloche with the value in words and payment clause "CINCO BOLÍVARES" / "PAGADEROS AL PORTADOR EN LAS OFICINAS DEL BANCO" «Payments to the Bearier in the Offices of the Bank», At right of the number "CUATRICENTENARIO DE LA FUNDACION DE CARACAS / 1567-1967" (Quadricentenary of the foundation of Caracas).
  • Reverse: Blue design. In each corner, the value "5" over varying gilloche. In the center, map of Caracas done in 1578 by Diego de Henares, at left Coat of Arms of Caracas, and at the right Coat of Arms or Venezuela. On the top, the issuer authority "BANCO CENTRAL DE VENEZUELA" «Central Bank of Venezuela», and on the bottom the value in words "CINCO BOLÍVARES".
  • Common name: This note was known as "Dieguitos".
  • Conmemotative motive: Quadricentenary of the foundation of Caracas, which was on July 25th 1567 by the captain Don Diego de Losada.
  • Designer or engraver: The design of the obverse representation of Diego was done by Felipe Sánchez, and the Simon Bolivar portrait was done by Alfred Sealey.
